Standard List of Documents:
The couple must provide the following set of documents:

Passports (International) - Copies of Birth Certificates.
This document must be translated into English, and the translation must be certified by a notary or a competent authority.
If you have ever changed your name (apart from marriage), you will need to provide documents confirming the name change (e.g., a name change certificate), if applicable. This document must also be translated into English and the translation certified by a notary or a competent authority. - Original documents from competent authorities in your country confirming your current marital status (single, divorced, widow, or widower), with a date, stamp, and signature. For most countries, this document must bear an Apostille or an equivalent stamp for some countries (registry stamp). The document must be translated into English, and the translation must be certified by a notary or a competent authority.
Note: Marital status documents are valid for 3 months from the date of issue. EU citizens are not required to provide an Apostille. Along with the translation, the original document is mandatory. - Original divorce certificate or court decree of divorce if you have had a previous marriage that has been dissolved. For most countries, this document must bear an Apostille or an equivalent stamp for some countries (registry stamp). The document must be translated into English, and the translation must be certified by a notary or a competent authority.
Death certificate of the previous spouse, if one of the future spouses is a widow or widower. This document must also bear an Apostille and be translated into English. - If one of the future spouses is under 18 years old, written consent from both parents is required, or if the father is deceased or unable to give consent, from the mother and/or legal guardian.
This consent must be prepared in English (it can be obtained from a District Court in Cyprus) or issued in the country of residence, certified with an Apostille, and translated into English.

Detailed Instructions for Israeli Citizens
To Get Married in Cyprus, Israeli Citizens Need to Provide the Following Documents:
Tamtzit Rishum - This document is issued by the Ministry of Interior (Misrad Hapnim).If you changed your last name upon aliyah (immigration to Israel), it is important to obtain the second page of this document, which specifies the name change.
The document must be translated into English, and an Apostille must be affixed to the translation.
Important: You can obtain an Apostille either from the Ministry of Foreign Affairs in Jerusalem or from a regular municipal court, which is twice as cost-effective. Apostilles issued by municipal courts are fully sufficient.
Birth Certificate
Some municipalities also require a birth certificate.
If you were born in Israel, your birth certificate is issued in English by Misrad Hapnim.
If you were born in another country, your birth certificate must be translated into English, and the translation must be certified by a notary or competent authorities.
Divorce Certificate (if applicable):
If you were previously married, you must provide a divorce certificate or a court decree of divorce.
This document must be translated into English, and an Apostille must be affixed to the translation.

To Get Married in Cyprus, EU Citizens Need to Provide the Following Documents:
Capacity to Marry (Multilingual Standard Form)
This is the primary and most important document.
It is issued within the EU for EU citizens and includes your personal details: name, surname, parents’ names, place of birth, and marital status.
This document can be translated into EU languages by simply selecting the required language, such as Greek, by ticking the appropriate box.
Important: The original document must be submitted. No additional translation or Apostille is required within the EU.
Birth Certificate
If your birth certificate is originally issued in English, a copy of it will suffice.
If the certificate is issued in any other language, it must be translated into English and the translation certified by a notary.
Divorce Certificate or Court Decree (if applicable)
If one of you was previously married, a divorce certificate or court decree must be provided.
The document must be translated into English and certified by a notary.
Important: EU citizens are not required to provide an Apostille for documents issued within the EU.
